• Worker's Comp Report Form 8AWC    

        Fill out this form after the injury- This is typically done with the employer at the time of the accident, but if it hasn't been done, use this form.

 

• Worker's Comp Medical Form 75 WCA-1

       Have your medical provider fill out this form after a visit.

VEHICLE ACCIDENT FORMS AND VIDEOS:

LOO LAW Accident Booklet - This booklet should be printed and put in every car you own. In the event of an accident, fill in this booklet and take pictures with your cell phone.

After the accident:

In the State of New Hampshire, any motor vehicle accident causing death, personal injury, or combined vehicle/property damage in excess of $1,000.00 must be reported in writing to the NH DMV within 15 days.

 

Print and fill in this form IF it was not completed at the accident scene.
